- Excluded Student Questionnaire
- An excluded student questionnaire is a questionnaire given to students who are initially selected to participate in an assessment, such as a standardized exam, but are then determined ineligible. Usually, students are excluded because the school finds them unable to participate due to limited English proficiency and/or disability, despite the use of permitted accommodations. The questionnaire allows for analysis on characteristics of excluded students.
