Individualized Transition Plans

Plans for the progression of students with disabilities, aged 14 and over, from secondary school to adult life - included as part of an Individualized Education Program, and intended to insure that students are prepared at graduation to participate in activities that interest them, such as postsecondary education, vocational education, employment, and independent living.
