The school guidance counselor should be able to assist you with the college application process and what is usually referred to as a "transitional IEP".  This will help your daughter continue with support , if warranted, at the college level.  

The counselor should be able to attach any necessary documentation to the schools that are receiving her transcripts.  Good luck to all during this exciting time!
