Ask him.  Again, it depends on his age--13 vs 18.  For some teens playing with dolls is exploring a girls world with which he may have had no contact.  for some, it is fantasy and purely heterosexual. And for some males, it is a manifestation of lack of social maturity. I would sit down with him and ask why he is playing with dolls.  If he can't give a satisfactory explanation, I would seek counseling.
