What the Expert Says:

There could be a variety of reasons why your child does not do well on her tests.  I would recommend that your first step is to ask the teacher what she sees are potential reasons so that you know how to help your child.  Based on what the teacher says and what the teacher does to help the children with testing problems, you can see how best to supplement and help your child.
Regardless of what the teacher advises, most children who have problems at this early age often have them because they either cannot read, do not understand what they read, they do not know how to follow instructions, or they do not know how to focus.

Repetition is your best bet specially if math is where she needs most help.  If it is basic math facts for example, use flash cards, fill ins, games, etc so that it makes her learning both fun and educational.

I agree with MTrejo. Asking your daughter's teacher is the first place to begin. She sees what happens in the classroom that might distract your daughter or confuse her. After you find whether it is a distraction or a skills related difficulty, you're better set to give your daughter advice or to find practice work at home.

MT's comment about reading is so important. Even with Math, reading can be the trouble especially if the instructions are above grade reading level. If her difficulty is with the Math facts, drills take time; but they really are the fastest shortcut. This site has printable Math flash cards, videos of the flash cards and practice Math worksheets:

Otherwise, it might be good to get a tutor. I know a friend who is a tutor and has an online tutoring site:

I feel your pain as I had a son who took forever to learn to read, but once he realized that he was already doing most of it well he excelled with extra practice. Practice helped everything.
