It sounds to me (as a professional) as if he has a learning disorder which is affecting his reading and written language skills. Often this isn't noticed until around fourth grade. As the workload increases, and he has to do more independently, he begins to fall behind. It would be a good idea for him to be assessed formally so that these issues can be addressed. Glasses often aren't enough to address ocular-motor weaknesses, and these, too, need to be evaluated by a paediatric Optometrist who could prescribe a specific visual therapy programme for him.
