Dyslexia is an umbrella term for several different problems.  The best thing you can do is find a specialist.  Many ophthalmologists are now becoming better able to do diagnosing.  If not, the doctor will be able to refer you to someone else.   Either way, the specialist will be able to do an evaluation, prescribe the best course of action (therapy, corrective lenses, etc) and you will have the documentation you need.
