What do I need to do to help an eleven year old boy with rage issues?

This child is a loving, respectful, intelligent, social and caring child but he has periodic outbursts of rage that are so diametrically opposed to his normal behavior.  He becomes destructive, will not speak but uses grunts/screams, clenches his fists, etc.  He claims to have no recall for these events after he has calmed down.  They obviously exhaust him and he usually sleeps very deeply afterward.  We have not been able to pinpoint exactly sets him 'off', and he will not talk about it.  I am hoping someone could shed a light on what the problem may be and what I can do to help him.  I am wondering if he needs psychiatric care and possibly medication.  Thank you so much.
