How can I help my first grader get over her fear of taking tests?

My 1st grader is a very poor test taker. She knows the information being asked of her on the test, but doesn't demonstrate that she knows what she's doing. You can take the test and ask her the same questions and she will answer them perfectly, but on the test, she gets the questions wrong. How can we help her overcome this issue?
