Giving your household income is not a requirement for admission to public school.  But if you hope to qualify for any assistance programs like free/reduced lunch than you would have to include your income on those application forms - and the school should definitley be able to assure you that that information will be kept confidential.

If the pre-k is a public program, offered through the public school district or a Head Start program then the answer to this question is yes.  Public pre-kindergarten and Head Start programs are federally funded and enrollment is based on a qualification process.  These programs are provided to the public free of charge and are offered to children who are considered "at-risk", meaning from low-income households.  In order to enroll your child in a program that is federally funded you will have to fill out a declaration of income.  As a veteran public pre-kindergarten teacher I can assure you that these forms are kept safe and not shared with anybody.
