Also if the rewards should be per week or per month or both or per point.

I have found that sticker charts work well. I use them, arrange it to were example: 3 stars is 30min extra before bed 5stars your chose of family movie 7stars all the above and your choose of meal and desert always even if star is not recieved let the child know that they that have to try daily to aim for the star and keep up good work.
