Has your child ever been retained?
What subject(s) is giving your child the most difficulty?
What does your child's teacher(s) say?  
What do they notice?
Does your child like school?
Has your child ever been tested? (learning disabilities or gifted)
Does your child have any physical problems?
Have you had your child's hearing and eye site tested lately?
Does your child have any trouble at home?
What do you notice about your child's learning?
When you teach your child something new, is he/she able to pick it up easily?
Do you know your child's learning style?
